PRIOR TO REGISTRATION:

1. TEAM ROSTER

• A copy of your certified roster must be presented at registration. Guest players can be written in at the bottom of the roster with names of absent players crossed out.
  
• Teams from outside the US must provide a team roster which includes name, jersey number, date of birth for each player. It must be signed and stamped by the local soccer association registrar. Each player on the roster must have a valid player book.
  
• Players’ jersey numbers MUST be listed on the Tournament Team Roster.
  
• Coaches should register at least 1-hour prior to your first scheduled game.
  
• Players may not be registered or play for more than ONE team during the tournament regardless of Age, Gender or level of play. Teams identified as breaking this rule will be dismissed from the tournament and no refund (partial or full) will be issued.
  
• You will be contacted via email regarding the tournament schedules and maps 1-2 weeks prior to the tournament date. The schedules and field map can be viewed under the tournament section of our website.

2. MEDICAL CONSENT FORMS

- You must submit a copy of a medical consent form for all participating players. This form can be found online at our website http://www.lancasterdepewsoccer.com/ . This must be presented at registration with the roster and passes for each player.

- Clubs that have their own completed Medical Consent forms / Manifests may use them as proof of consent.

3. PERMISSION TO TRAVEL FORMS

- All teams from outside the New York State area are required to submit an approved Permit to Travel form. You can apply for them within your area organizations. Please contact your organizations governing body for more information. (may require 30 days to process, apply early)

AT REGISTRATION:

- Bring your LDSC Tournament roster (5 copies), player passes, permission to travel form (if applicable), medical consent and release forms / manifest to the registration area 45 min prior to the start of your first game.

- Registration for teams playing on Friday July 30^st, (or Sat) will be available starting at 5:00pm behind Lancaster High School. All other teams may register on Saturday July 31^st between 7:00am – 2:00pm behind Lancaster High School.
